How to get a record of your COVID-19 vaccination

 

Proof of your COVID-19 vaccination will be recorded on your immunisation history statement after you’ve had your vaccine.
 

Once you have received all required doses, you will also be able to access your COVID-19 digital certificate.
 

You can get proof of your COVID-19 vaccination through:

Medicare online account

Express Plus Medicare app

My Health Record

Individual Healthcare Identifiers Service

Australian Immunisation Register
 

 

Medicare online account

1. Sign in to your myGov account (my.gov.au) and choose Medicare.

2. From the Immunisation history section, select View immunisation history

3. Choose your name.

4. Click on View immunisation history statement (PDF) or View COVID-19 digital certificate (PDF).

Express Plus Medicare app

1. Open the app and sign in.

2. From the Services section, choose Immunisation history.

3. Choose your name.

4. Click on 'View immunisation history statement' (PDF) or View COVID-19 digital certificate (PDF)

My Health Record


You can also get proof of your COVID-19 vaccinations from My Health Record.

If you already have a My Health Record:

1. Sign in to your myGov account (my.gov.au) and choose My Health Record.

2. Open your My Health Record.

3. Access proof of your vaccinations from your Record Home page or from your Immunisations view page (select Documents, Immunisations).

If you don’t already have a My Health Record, but you have a Medicare card:

1. Sign in to your myGov account (my.gov.au) and link to My Health Record.

2. Verify your identity and create a My Health Record, making sure to select ‘Yes’ to include immunisations information (Medicare information) in your My Health Record.

3. Open your My Health Record.

4. Access proof of your vaccinations from your Record Home page or from your Immunisations view page (select Documents, Immunisations).

 

FAQs


Can I enter a Mounties Group venue with only one dose of the vaccine?

Only fully vaccinated patrons will be permitted entry into Mounties Group venues from October 11, as outlined under the NSW Government legislation.
 

What If I am unable to obtain a vaccine?

Under the Public Health Act 2010, patrons who are unable to be administered with approved vaccines will only be permitted entry into our venues if they obtain a COVID-19 Vaccine Medical Contraindication

This document is the only approved medical exemption we are able to accept.
